Our Commitment:
Serving as a link between the Hospital and the community, the Social Services Department at Bloomsburg Hospital helps patients build upon the success of their inpatient care after they leave the Hospital.
The primary function of Social Services is to plan continuing care. Before a patient is discharged, their condition is evaluated to determine what specialized medical care or community agency services may be needed. Based on this evaluation, services ranging from home health intervention to nursing home placement can be appropriately coordinated.
Convenience:
A member of the Social Services department conducts evaluations right in the patient’s own room. If continued care or local agency support is needed, the Social Services representative will help coordinate the care and insurance coverage.
