Description
A clinically competent Caseworker who works as a member of the Treatment Team in providing evaluation and recommendations for approximately twenty hospitalized patients. Patient population ranges from adolescence to geriatrics. Possess the ability to interact with the Hospital and Community population in accordance with the individual's age group and level of growth and development. Coordinates utilization of appropriate resources to meet patient's needs. Provides assistance to patients and their families in coping with the emotional impact of mental illness, alcohol and drug abuse. The Caseworker provides post-discharge planning to ensure continuity of care, performs other related duties as assigned by Psychiatrist, Nurse Manager and other members of the Treatment Team when indicated.
Hours
Hours are from 8:00am to 4:00pm Monday thru Friday. May Include other hours as needed for department staffing.
Experience
Educational requirements include a Bachelor's Degree in Social Work, Sociology or other Human Service related field.
Experience with Mental Health patients and/or in acute psychiatric setting preferred.
Must possess excellent communication skills to deal effectively with patients, family, hospital employees and the public.
Current BLS preferred.
