Description
Join our team of professional RNs dedicated to implementing the Nurse Family Partnership Program for first time mothers.
The Nurse Home Visitor acts as a case manager and more! This position implements standard protocols
for families enrolled in the Nurse-Family Partnership program.
A BSN and PA RN license are required.
Client coverage area is primarily Northumberland, Union and Snyder.

Experience
Possesses a license to practice as a registered nurse in the Commonwealth of Pennsylvania.
Possesses a current Pennsylvania driver\'s license.
Must have access to independent transportation to comply with standards of job performance.
Must be able to communicate effectively.
Baccalaureate degree nursing program.
At least one year of nursing experience.
