The RN Case Manager coordinates the implementation of the plan of care which integrates the interdisciplinary care team, and focuses on holistic care.
Hours
Days: with weekend/holiday rotation
Experience
RN positions must have valid PA RN license plus a minimum of one year nursing experience. All applicants must be caring, compassionate, organized, communicate effectively, and possess current PA Driver’s License.
