Columbia Montour Home Health and Hospice provides special services
for specific needs, including:

 

 

Home Health
Provides professional services prescribed by your physician for you following illness, injury, or surgery to be delivered in the comfort of your own home.

 

Hospice
Provides care for terminally ill patients including skilled nursing, home health aides, social work counseling, spiritual care and trained volunteer support in a variety of settings (home, nursing home and Columbia Montour Home Hospice at Maria Hall).

 

Bereavement Camps
Helps children learn how to cope with the strong emotions of grief.

 

Nurse-Family Partnership
Helps first-time parents succeed.

 

Rehabilitation
Includes nurses, therapists and home health aides who work as a team with the patient to restore body strength and function.

 

Advanced Technology Nursing
Provides in-home care and instruction for patients of all ages and includes peripheral venous access devices, central venous access devices, IV antibiotic therapy, IV pain management, enteral nutrition, total parenteral nutrition, ventilators, tracheostomy care and oxygen therapy.

 

Diabetes Education
Teaches both Type I and Type II diabetics and family members about diabetes and their prescribed treatment, methods of monitoring and diabetes-related complications.

 

Transitional Care
Provides care to patients with life limiting illness who are not yet ready for Hospice or who could return to wellness in the community.

 

Obstetrical Care
Includes early maternal discharge, postpartum follow-up and education, high-risk antepartum and lactation education to assist breast feeding mothers.

 

Pediatric Care
Includes premature infant care, phototherapy for newborns, nutritional support, and rehabilitative services.

 

Home Care Monitoring System
Provides daily at-home monitoring of vital signs for patients with various health conditions. The system gives the users the comfort and independence of staying at home and the security of daily monitoring and follow-up medical attention if necessary.
