Nurse-Family Partnership
Making a difference for a lifetime.

The Nurse-Family Partnership is a home visitation program for low income women who are having their first baby. A registered nurse with advanced education will visit throughout pregnancy and continue to visit until the baby
is two years old.
The goals of the Nurse-Family Partnership program are to:
1. Improve mother’s health during pregnancy
2. Promote healthy child development
3. Promote self-sufficiency of families
The Nurse-Family Partnership is one of 11 research-based blueprint prevention programs in the country as identified by the Center for the Study and Prevention of Violence at the University of Colorado. Twenty-three states in the United States and 40 counties in Pennsylvania have Nurse-Family Partnership sites.
Columbia Montour Home Health Services/Visiting Nurses Association’s site provides home visitation to families in Columbia, Lower Luzerne, Montour, Northumberland and Snyder Counties.
